Job Description

This member of the PACT multidisciplinary team is responsible for conducting nursing assessments, assessing physical health needs, making appropriate referrals to community physicians, providing management and administration of medication in conjunction with the psychiatrist, and support services.

Responsibilities:

• Under the direction of the team psychiatrist and in collaboration with other registered nurses on the team develop, revise, maintain, and supervise team psychopharmacologic and medical treatment, including transcribe, prepare, administer, evaluate, and record psychotropic medications prescribed by the team psychiatrist. 
  • Organize and manage the system of getting medication to clients, manage pharmaceuticals and medical supplies, integrate medication administration into clients’ individual treatment plans and provide medication education.
  • Coordinate, schedule, and administer medical assessments of client physical health by making appropriate referrals to community physicians for further assessment and treatment to coordinate psychiatric treatment with medical treatment.
  • Evaluate and chart psychotropic medication effectiveness, complications, side effects and arrange for required lab work according to protocol.
• In collaboration with team lead and other registered nurses on the team, develop, lead and maintain relevant educational groups for PACT clients. 
• Provide service coordination (Case Management) for an assigned group of clients including coordinating and monitoring the activities of the Individual Treatment Team (ITT).
• Assume primary responsibility for developing, writing, implementing, evaluating, revising overall treatment goals and plans in collaboration with the client and the ITT on assigned consumers.
• Providing individual supportive therapy and symptom management, ensuring immediate changes are made in the treatment plans as client needs change, educating and supporting clients, families, and advocating for clients’ rights and preferences.
• Participate in and conduct parts of the client-centered comprehensive assessment of the following:
  • Psychiatric history (i.e. onset, course and effect of illness, past treatment and responses, and risk behaviors).
  • Mental Health status and diagnosis.
  • Physical and dental health
  • Use of drugs or alcohol, social development and functioning.
  • Activities of daily living (i.e. self-care, living situation, nutrition, money, management, family structure and relationships)
• Consult with community agencies and families to maintain coordination in the treatment process.
• Provide on-call crisis intervention covering night time and weekend hours.
• Document client progress to maintain a permanent record of client activity according to establish methods and procedures.
• Participate in daily staff organizational meetings and treatment planning review meetings.
• Take the lead role or participate in providing medication administration and medical services.
• Participate in treatment, rehabilitation, and support services.
• Perform other duties as assigned.

Physical Requirements:

This position normally requires that physical demands of standing, walking, bending, lifting or performing other work requiring low physical exertion, talking and hearing on a regular basis to perform the job requirements. These physical demands are required up to 80% of the time. Position also requires ability to drive with adequate vision and skill.

Non-Physical Requirements:

Education:

• Nursing degree (associates, bachelor's, or master's) required. 

Experience:

• 1 years’ experience in an inpatient or outpatient psychiatric setting, preferred.

Special Skills:

• Electronic Medical Records skills (EMR).
• Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illness, and respect for client rights and personal preferences in treatment are essential.
• Good interpersonal skills.
• Possess and utilize effective verbal and written communication skills.
• Ability to provide service in a culturally competent manner.
• Ability to be self-motivated, work independently, and as a team member.
• Ability to represent the agency in a professional manner within the community.
• Bilingual (English/Spanish) preferred, but not required.

Licensure, Registration, Certification:

• Must be licensed to practice as a registered nurse within the State of Washington.
• Valid WA driver’s license, assess to vehicle and minimum required liability insurance for WA State.
• Must be deemed insurable as determined by Catholic Charities’ liability insurance provider.

Employment is conditional upon:

• Being cleared by criminal background check and fingerprinting when required.

Wage Range : $84,800 - $90,100, depending on experience.

Signing Bonus : $10,000 paid in two installments with a 27-month total commitment.

Work Schedule : Tuesday - Friday, 7am to 6pm
