We are looking for a Nurse Practitioner to join our Clinical team. The Nurse Practitioner will conduct Health Risk Assessments for Medicare, Medicaid, and Commercial Marketplace members/patients of primary care physicians, medical groups, and the health plans we serve. The Nurse Practitioner will perform comprehensive assessments that are compliant with the annual wellness visit in the patient's home.
The position is based in Camden, Arkansas, and surrounding counties (Fordyce and Rison), and will require the applicant to have reliable transportation to travel to the patient's home.
